To complete the Take Stock in Children of Sarasota County application, you will need the following documents:

Please note the following documents are not accepted for income qualification:
Financial Eligibility Documentation (Section G)
In Section G of your application, you’ll need to upload documentation verifying your financial eligibility.
Accepted Documents
If using your 2023 1040 tax form: Student must be listed as a dependent and the total income must be reported.
If qualifying through Medicaid or SNAP: Upload a copy of your Eligibility Letter or Benefits Summary Page from the DCF Access website.
Please note that Medicaid cards alone will not be accepted. For reference, view example.

Social Security Card
Please Upload a photo of the student’s Social Security card.

Additional Helpful Information
Here are a few clarifications to assist you with completing the application:
Student Email: Use a personal email address rather than a school-issued email to ensure consistent communication with Take Stock in Children.
Resident Alien: Refers to a permanent legal resident (page 1).
Entry Grade Level: This is the student’s current grade level (page 2, Section A).
Parent/Guardian Social Security Number: This field is optional and not required.
